IMPORTANT! The Process Has Changed! We no longer email packets to REC Centers!

ALL  Merchant Mariner Credential applications, submit directly to NMC by clicking the below button.  Please send any documents in Adobe PDF format only, and include your Last Name, First Name, Mariner Reference Number (or last digits of your SSN)  in the subject line of your e-mail.  Submit to NMC

Kim’s blonde-proof checklist forUSCG NMC Original Application

  1. Complete the course. Put diplomas in application packet. Diplomas must be dated within one year.  Please do not include online course completion certs. 
  2. Complete a USCG Approved CPR/1st Aid/AED in person course. We normally have one the week after OUPV class. Add diploma to packet. Diploma must be dated within 1 yr AND HAVE USCG COURSE CODE ON IT if not our class! Please do not include online course certificates in packet
  3. Get your Transportation Worker Identification Credential (TWIC) To make appointment:  https://universalenroll.dhs.gov  Please put the receipt you are given at the end of the appt. into your packet or front/back photo of TWIC card.  Receipt is actually preferred) 
  1. Get a physical filled out on formCG_719K PhysicalBe sure to fill out all little name boxes on top/bottom of pages and on page 3, check ‘other’ and write: ‘MMC Application.’  When you are ready to email packets in, the medical will be emailed seperately to: medaip@uscg.mil  with full name in subject line.
  2. Compete a drug screen on form –CG_719P Drug.  You may also use a drug consortium letter dated within 180 days.  We are also a drug consortium.
  3. Fill out sea service formCG_719S Sea Service Be sure to include proof of ownership if boat belongs to you or family member.
  4. Fill out application for license on form-CG_719B Application.
  5. If you answer yes to anything in section Section III (2) (Convictions and Drug Use), please fill out formCG_719C Conviction
  6. Complete application fee payment to the government pay.gov (see picture below for fee info).Very Important!!! Email receipt to yourself and then print the email and put in the packet.
  7. Send in your application or have us act as your third party agent! Send via pdf by email only to MMCApplications@uscg.mil

      ALL Merchant Mariner Credential applications, submit all your forms (except the physical but do include the drug screen) and supporting documentation directly to NMC at: MMCApplications@uscg.mil.  Keep the pdf to under 15 MB in size. 

  8. Email completed medical (just the physical) form to:   medaip@uscg.mil  w/ mariner’s full name in the subject line.

Kim’s blonde-proof checklist forUSCG NMC Renewal Application

  1. Fill out application for license on form- CG_719B Application. Be sure to check box for Renewal on bottom of page one and write license held in box below. If you are upgrading, write license you are requesting.
  2. If you answer yes to anything in section Section III (2) (Convictions and Drug Use), please fill out form – CG_719C Conviction.
  3. Fill out sea service form –CG_719S Sea Service If vessel is owned by you, include proof of ownership (title, registration, etc.) You need 360 days in the last 5 yrs.
  4. Get a physical filled out on form – CG_719K PhysicalBe sure to fill out all little name boxes on top/bottom of pages and on page 3, check ‘other’ and write: ‘MMC Application.’  When you are ready to email packets in, the medical will be emailed seperately to: medaip@uscg.mil  with full name in subject line.
  5. Compete a drug screen on form –CG_719P Drug.  You may also use a drug consortium letter dated within 180 days.  We are also a drug consortium.
  6. Renew your Transportation Worker Identification Credential (TWIC) if necessary, or check the TWIC exemption box on top of page 2 of application. (You must renew TWIC if you have 200GT Master license or higher or go thru a secure port.) To make appointment:  https://universalenroll.dhs.gov Please put the receipt you are given at the end of the appt. into your packet or front/back photo of TWIC card.  Receipt is actually preferred)
  7. Complete application fee payment to the government Pay.Gov (see picture below). Email receipt to yourself and print to put in packet. Basic Renewal:If Renewal with Raise of Grade, fill out as follows:

Email in your application packet (all forms except for 10 page medical) send via pdf by email only to: MMCApplications@uscg.mil  ALL Merchant Mariner Credential applications, submit all your forms (except the physical but do include the drug screen) and supporting documentation directly to NMC at: MMCApplications@uscg.mil.  Keep the pdf to under 15 MB in size. 
  9. Email completed medical (just the physical) form to:   medaip@uscg.mil  w/ mariner’s full name and reference # in the subject.
